ReBoot Forms

Reboot Required Forms:

The following forms are required for any church group attending ReBoot:

1. Reboot Participant List: all students and adults must be listed on your church’s roster. Participants Lists are due two weeks prior to your session date (e.g. churches attending the Jan 13-15, 2023 session will submit their lists by Dec. 30).
2. Participant Release Form: required for every individual student and adult attending the event. Due at check-in.
3. Statement of Compliance: steps must be taken by your church to complete this form prior to your arrival at camp. Due at check-in.
