Participant Release Form
Required for every camper and adult chaperone present at camp.
Background Check Form
Required for every participating church. We ask all adults attending camp to pass a background check. See form for details.
Youth Group Roster
A completed roster listing all campers and adult chaperones should be submitted 14 days prior to your session date.
Certificate of Liability
Required for every participating church. Provided by your insurance company, a Certificate of Liability lists your policy and coverage information. The certificate should name Crossings Ministries as a Certificate Holder.
Mission Track Volunteer Waiver
For those participating in Missions Track only (not needed for Adventure track churches). Required for every camper and adult participating in off-campus missions activities.
